List of documentation to be submitted 

• Completed application form
• A copy of the potential beneficiary’s identity document
• A document confirming the payment of the co-financing amount
• A warranty document for the energy-efficient stove (the warranty conditions must be valid for at least 2 years from the date of signing the application form) issued by the manufacturer and/or supplier, which must mandatorily include the following information:
  •  Details of the parties;
  • The name, model, and serial number of the energy-efficient stove;
  • The total value in national currency;
  • The bank details of the manufacturer.
    
    
• Certificate from the LEPL Social Service Agency or the LEPL Agency for IDPs, Eco-Migrants and Livelihood Provision regarding the receipt of the following types of assistance (if necessary):
  
  • State pension;
  • Social package;
  • State subsidy;
  • State compensation for persons with disabilities;
  • Subsistence allowance;
  • Assistance for persons permanently residing in high-mountain settlements;
  • State compensation granted after the death of an employee;
  • Social assistance for damage caused by health impairment during the performance of labor duties;
  • Social assistance for persons employed in underground mining operations (mines and quarries);
  • Persons with the status of internally displaced persons;
  • Families affected by a natural disaster and facing the threat of relocation.

• Certificate from the local municipality regarding the location of the village where the energy-efficient stove is to be installed in relation to the dividing line (if necessary);
